Click here to get a copy of the Expense Reimbursement Form. There are two forms on a letter size sheet of paper. When printed, cut in half and submit one form with receipts for reimbursement. Please note, some receipts may not mention HST (or GST) and simply say "TAX". Since July 1, 2010, out-of-province organizations charging HST(GST) must charge the tax rate of the province being sold to. (For example, for items sold to BC, Concordia Publishing House in the USA charges Canadian HST at a rate of 5% for books, and 12% for everything else. But their invoice will simply say "TAX".) |
